We can not take prescription orders over the phone.

You will receive a re-order slip each time a repeat prescription is processed. Please only tick those items you require. Please do not over order medication.
There is a box available at the front door for completed re-order slips - do not enter the Health Centre.
During a consultation with your doctor, please do not ask for repeat medications as this is not the best use of the time spent with your doctor.
If a medication you need does not appear on the re-order slip and if your doctor has indicated this may be ordered as a repeat, please write this clearly on the bottom of the re-order form or use the repeat prescription request forms available at the reception desk.
Due to the coronavirus pandemic, prescriptions are not available for collection at reception - they will be sent directly to your registered pharmacist.
If you have not registered at a pharmacy prescriptions will be sent to your local pharmacist. This can be determined by the third digit of your postcode:
